If you are a new patient of Fialkov Chiropractic, please take a minute to download this new patient form, print it, and fill it out before you come in.

Location
The office is located at 279 S. Yonge Street (US1), Ormond Beach, FL 32174.
In the Daytona Beach area, take I-95 Exit 268 (Route 40, Granada Blvd). Take Granada Blvd. east 4
miles to US 1, S. Yonge St. in Ormond Beach.
Turn right on S. Yonge St. We are about 1/2 mile south on the left, directly north of Division Ave.

Walk-ins are welcome but evenings and
Saturday are by appointment only.
